🛡️ Terms of Service for Prideful Medport LLC

Effective Date: 11/01/2025 

By using Prideful Medport LLC’s Non-Emergency Medical Transportation (NEMT) services, you agree to the following terms:

1. Services Provided We offer scheduled transportation to medical appointments, pharmacies, grocery stores and healthcare facilities. Services are available Monday–Friday, 5AM–7PM.

2. Eligibility You must be a registered client, caregiver, or authorized Medicaid recipient to use our services.

3. Scheduling & Cancellations Appointments must be scheduled at least 48 hours in advance. Cancellations must be made at least 8 hours before pickup time.

4. Communication You may receive SMS or email reminders, updates, and service alerts. Reply STOP to unsubscribe. Msg & data rates may apply.

5. Liability We are not liable for delays caused by traffic, weather, or third-party providers. We are committed to safe, timely service.

6. Privacy Your personal and health information is protected under our Privacy Policy and HIPAA guidelines.
